TIME TO ARRIVE
Your first visit will involve a one-hour evaluation consultation with the physical therapist. We ask that you arrive 15 minutes early to fill out paperwork and insurance forms.
WHAT TO BRING
Please bring your physician prescription or referral, your insurance card, your ID or driver’s license, your primary registration forms and co-pay, a list of your medications, and a change of clothing, if desired.
WHAT HAPPENS NEXT
Following your initial physical therapy evaluation, the physical therapist will determine a plan of care and a treatment program customized to your needs. Based on your diagnosis, your subsequent visits will focus on achieving your treatment goals.
Our front office will schedule you for subsequent appointments. We are flexible and will work your physical therapy appointments around your work and school responsibilities.

INSURANCE & BILLING
We accept most insurances and will work with your insurance company for you so that you can focus on recovery and health. Please call our clinic if you would like to confirm that we accept your insurance coverage.
If your provider requires a co-pay, we will request that payment at each visit as this is an insurance requirement.
